  Holy Baptism is the basis of the whole Christian life, the gateway to life in the Spirit, and the door which gives access to the other sacraments. Through Baptism we are freed from sin and reborn as sons of God; we become members of Christ, are incorporated into the Church, and made sharers in her mission; Baptism is the sacrament of regeneration through water in the word. - Catechism of the Catholic Church 1213
  Parents who are seeking Baptism for their children are asking that their child be initiated into the Catholic Community and the local Church Community of Holy Name. The Church baptizes infants/children on the faith of the parents. The idea is that parents have a deep faith, which they actively live out in the experience of the Catholic Church Community. Children's faith is learned from their parents. We consider children from birth through the age of six to be infants. Children 7 and older who need to be baptized must attend religious education classes.
  
  Our parish requires that parents and godparents attend two baptismal sessions. Godparents who live out of town must attend the baptism session offered at their home parish and bring a certificate of attendance.
  
  Godparents must:
  - Have received the sacraments of Baptism, Confirmation, and Holy Communion.
  - Be at least 16 years of age.
  - If a couple, be living in harmony with the Catholic faith.
  - If a single person, be single or widowed and not living in a free union.
  Holy Name celebrates baptisms bi-monthly. Our next scheduled baptism date is February 9, 2025 and classes will be held on January 30 and February 6 at 6:00 pm in the Rectory. Please fill out the form below to register.

  Godparents-- One godparent is required for Baptism, but you may have two (one male & one female). A godparent must be an actively practicing Roman Catholic, fully initiated and at least 16 years of age. One baptized non-Catholic Christian may serve as a Christian Witness to the Baptism along with the Catholic godparent. Record the names & parish affiliations of your chosen godparents below. Be aware, by providing the names and status of Godparent or Christian Witness, you are attesting to the eligibility of each person to serve in the designated role.   Video Permission Release-- Various Masses are recorded at Holy Name Parish and live-streamed/posted on Facebook. Therefore, it is possible your child could be recorded during Baptism or pre-Baptism anointing. By completing this Baptism Registration Form, you are providing consent for your child to appear in the video. Furthermore, you are certifying that you are legally capable of executing such an authorization.
